Drs. John Rodgers and William Preskenis partner with Touchpoints Rehab to create dynamic pulmonary care program
June 26, 2017Touchpoints at Bloomfield entered into a partnership designed to bring continuity in pulmonary care from the acute hospital setting into the post-acute, skilled nursing and rehab care setting. The program will address repeat hospitalizations by applying integrated pulmonary care and sleep medicine consulting to many diagnoses, including COPD and congestive heart failure.
Sheri Shaw joins the iCare team as the new Transitional Care Nurse. She will be working on Touchpoints’ signature Congestive Heart Failure rehabilitation program with a focus on optimal care, minimal re-hospitalization and re-admission avoidance, meeting length of stay goals and transitioning patients between sites of care.
